Complete the Information Sheet and Acknowledgement form below.

Children 10 – 15 require direct Parent/Guardian supervision during all volunteer activities at South Pacific County Humane Society.  Parent/Guardian are required to volunteer side-by-side with their child.  Children 16 – 17 with Shelter Manager approval may volunteer without Parent/Guardian supervision. Children 10 – 17 Parent/Guardian must sign a Minor Consent and Release on Behalf of Minor form at the shelter in front of staff/volunteer.
