I am open to adopting a dog with special needs: Describe the general activity level in your home (very active, enjoy long beach walks, laid back, etc). Please be specific: How much and how often will you regularly exercise your dog? How will you supervise your dog's outdoor activity? How many hours each day will your dog be alone? Where will your new dog stay when nobody is home? Where will your new dog sleep at night? If your dog has a potty accident in your home, what type of correction do you plan to use? Please describe family, friends and children that are frequent visitors to your home including ages of children. List animals who frequently visit your home. Please include breed, age and their personality/behavior. Do your neighbors have dogs? Are you willing to work through your new pet's issues, if any? Are you aware that a new environment may be stressful for your pet, and they may exhibit uncharacteristic behavior? Do you understand that a dog can be an 18 year commitment? How much do you expect to spend on this dog per year?
Please consider food and supplies, vet costs, grooming, boarding, etc.
Have you ever given up a pet? Under what circumstances would you not keep your new dog? Please consider all possible future situations: financial status, moving, health changes, job changes, children, etc. If you become unable to care for your dog, what is your plan for the care of your animals?

Is there anything else you'd like us to know? I am 18 years or more of age. I agree the animal I am adopting will reside in my home as a pet. It is solely my responsibility to provide adequate food, water, shelter, training, affection, safety and medical care. SPCHS is in no way liable or responsible for any damage, accident or injury resulting from the placement of a dog into my household and I/we agree to indemnify and hold harmless SPCHS and its officers, members, and/or agents from any claims for damages arising from actions of a dog adopted by me/us. SPCHS may in the future contact us to check on the dog adopted. If for any reason, we are no longer able to provide a home to the dog adopted through SPCHS, we shall immediately notify them of the situation and make arrangements to return the dog to SPCHS.
